What is the title of this film with a black teenage football coach and player?

I saw it a few years ago. I remember the old coach had an affectionate relationship with his mother. He worked hard to get this bunch of young men to apply themselves to play High School Football. I do not think they were any high school team during the related story. They tried to prove the company that they move beyond their lives of crime. The coach took her out of the hall and youth from the streets and away from gangs and drugs and got them focused on teamwork and a positive self-esteem. I think the coach mother died during the film. I am not sure whether the film is based on a true story.
